The Institute of Medicine's report on medical errors first published in late 1999 initiated the patient safety movement. (1) This was followed by the Institute of Medicine's recommendations for dealing with this crisis in health care. (2) Although there was heavy emphasis on medication errors, scrutiny has since been applied to all fields of medicine. Surgical pathology errors have been highlighted in many recent reports of wrong site or wrong patient surgery or therapy because of surgical pathology error.3,4 In response, accrediting agencies such as the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) reevaluated their accreditation standards and effected standards with emphasis on patient safety. In addition, the JCAHO has implemented patient safety goals that must be addressed by all institutions accredited by it.5 Several of these patient safety goals apply to laboratory services including surgical pathology. (6) These patient safety goals are listed in Table 1. The College of American Pathologists (CAP) Laboratory Accreditation Program also strengthened its standards with respect to patient safety and initiated its own laboratory patient safety goals. (7,8) Although the CAP's patient safety goals mirror the JCAHO's patient safety goals, they are more specific for the laboratory and also offer specific guidelines for implementation of these goals. The CAP's patient safety goals are listed in Table 1. The CAP further suggests either that patient safety activities be placed under the overall auspices of the laboratory quality assurance (QA) committee as a subcommittee or that specific patient safety monitors be created within an existing QA plan.
